Learning Access Plan Builder

Build the shared plan, send a draft to trainers, then send the final plan as a PDF. No student information is kept here: to come back to a plan, or to read trainers' feedback, open the PDFs with "Open, import or read returned PDFs".

All plan builders
Or drag and drop PDFs here: a plan saved from this builder, returned trainer feedback, or an older pre-builder LAP.

1Plan details

Enter the name the student wants used, first name only. It fills in across the whole plan.

Accessibility Services contact

2In the student's words

Optional. Two to four sentences the student is happy for trainers to read: what helps them learn, how they like to be contacted. Write it with them, in their voice. Leave diagnosis out.

3Reasonable adjustments

One entry per barrier. Describe what might be challenging in this course, as a situation rather than a condition (for example, "When instructions are only given verbally…"). Then say exactly what will happen, who does it and when. Mark an entry as being confirmed if something still needs checking. Everything else applies straight away.

Common wording fills in all four parts for you, with the student's name. If only one trainer is listed in step 6, their name goes in too. Edit it to fit this student and course. Anything in square brackets needs filling in, and some entries come with an officer note that isn't printed.

4Only if it applies

Placement adjustments

Picking an adjustment fills in the wording with the student's name. Edit it to fit. Anything in square brackets needs filling in or deleting.

Placement points to discuss

Read each point to the student and tick it once you've discussed it. They print on the plan. The final plan can't be sent until every point is ticked.

Transition check-ins and notes stay in the DTSO's own records. Only what a trainer needs to know goes in the plan.

    5Referrals

Tick any service you've talked about with the student. Ticked services appear in the plan with how to book. Services that aren't ticked don't appear.

Accessibility Services refers directly. Conversion takes time and specialist resources, so refer only when the student can't use standard materials, even with the usual tools. A converted version that would be easier or nicer to have isn't enough on its own.

    Format needed

    6Who receives the plan

List the staff who need the plan to put adjustments in place. Pick from the staff list or type a new name and save it. Each card shows which adjustments name that person, so you can see who has something to do. The student's agreement to share with each person is recorded in step 7.

They receive the final plan with the student's email. The student's agreement is still recorded in step 7, including for students under 18.

    7Privacy conversation

Go through this with the student before anything is emailed. Nothing can be emailed until every point is ticked and their answers are recorded.

Permission for trainers to contact Accessibility

Read this to the student and record their answer:

Who the student agrees can receive the plan

Only people and services ticked here will be put on an email. Untick anyone the student doesn't want to have it.

Record the conversation in the student's case notes as usual. This page doesn't keep it.

        Signatures

The Accessibility Officer's saved signature is applied automatically. Signatures only appear on the final plan. If the plan changes after it's signed, it needs applying again.

        8Check and send

          Draft for trainer input

Send the draft to the staff listed so they can check the adjustments that name them. The draft has a feedback box under each adjustment. When staff send it back, open the returned PDFs with "Open, import or read returned PDFs" (you can pick several at once) and their comments appear under each adjustment.

          Final plan

          To trainers

The final plan with a note asking staff to check the actions with their name.

          To the student

The final plan with a short note to the student.

          To both

One PDF and one email to the student and staff together.

          To support services

The final plan to referral services the student agreed to share it with.

Each button saves the PDF, then opens a new email in your mail program. Email programs can't attach files from a web page, so attach the saved PDF yourself. Save a copy of the final plan to the student's record.

          Privacy: nothing is saved here

Nothing can be emailed until the privacy conversation (step 7) is complete, and emails only go to people and services the student agreed to. Student information stays in this open page only. It is never stored in the browser, sent to a server or kept between visits, and refreshing or closing the page clears it. To come back to a plan, open the PDF you saved with "Open saved or returned PDFs": the plan details travel inside that file, which lives with your email and records, not in this tool. The only things this page remembers are the course and staff lists, which hold no student information.
