2Who and why

Use this form for specific, time-limited sharing, such as asking a GP for information or talking with a school. It isn't needed to send a learning access plan to teaching staff.

How

Keep it as short as the purpose needs. Twelve months at most.

4Explain and sign

Read each point to the person giving consent and tick it once you've discussed it. They print on the form.

    Signatures

Sign with a mouse, finger or stylus, or type a name. If the form changes after signing, it needs signing again.

    Student

    2The practitioner (optional)

If you know who will fill it in, their name goes on the form. Add an email to send it to them directly.

3Talk it through with the student

Read each point and tick it once discussed. What the student agreed to prints on the form, so the practitioner can see it.

      2The caring role

For students who care for someone with a disability, medical condition or mental illness, or who is frail and aged, as defined by the Carer Recognition Act 2010. The person's own health care professional fills in the form.

3Talk it through with the student

Read each point and tick it once discussed.

        5Check and send

          When it comes back

Use Open a saved or returned PDF at the top. If the practitioner filled it in on screen, the answers appear beside the form for you to check. Printed and handwritten forms can't be read automatically. Nothing from a returned form is kept here.

          Privacy: nothing is saved here

Student information stays in this open page only. It's never stored in the browser or sent anywhere, and refreshing or closing the page clears it. Each form travels inside the PDF you save, so you can reopen it later. The only thing this page remembers is the Accessibility Officer list.
